To sum up, curing astigmatism is possible thanks to several kinds of surgeries available.
The most common astigmatism surgery is the one that uses the excimer laser, that can cure astigmatism and myopia at the same time.
It has been recently perfectioned a new surgery with the Femto Laser, that increases the chances of success with the excimer laser or remove a thin layer of corneal tissue to correct astigmatism, with very promising results. Astigmatism surgery with the excimer laser is mainly based upon two methods: LASIK and PRK. Today there are several variations of these two techniques.
The main surgery for high degree astigmatism is the implant of a intraocular lens inside the eye, or phakic lens. The latest news in correcting astigmatism are the High Tech intraocular lenses that represent a key solution for patients that have already undergone laser surgery and then experienced cataract. Another astigmatism and high degree myopia surgery consists in substituing the lens with a techniques called Refractive Phacoemulsification .
LASIK (LASer-assisted In situ Keratomileusis) with the Excimer Laser, is the most performed astigmatism surgery in terms of refractive surgery in the USA.
Its popularity comes from the advantages for the patients: almost no pain at all and great vision quality from the day after surgery. This kind of surgery requires a special tool called microkeratome to create a thin circular layer (flap) on the corneal surface.
The thin layer is flipped and placed on a side. Once the inner cornea portion is exposed, it’s time for the excimer laser ray to remodel it, correcting the refractive flaw. Finally, corneal tissue is replaced with no need of suture since tissues have a great adherence right a minure after this procedure. This technique is used to correct astigmatism, myopia and even hypermetropia.
This eye laser surgery is technically complex, but it by far the most comfortable for patients that want to treat myopia. This laser surgery has higher costs if compared to PRK.
Different from LASIK surgery, this method uses an instrument called FEMTO LASER to cut a thin circular layer (flap) on the corneal surface.
This layer is flipped and placed on the side. Once the inner cornea portion is exposed, it’s time for the excimer laser ray to remodel it, just like in traditional LASIK. Dr. Badalà has been one of the first to ever perform this surgery in Milan in 2008.
The cost for this laser surgery is higher than all the others previously described, because of its tools’ complexity and the high precision required.
PRK (Photo Refractive Keratectomy) is a surgery that uses the excimer laser and it is one of the most common in Europe.
It is performed with an excimer laser that emits an ultraviolet ray of light with a very high specific energy, but low penetrating power in biological cells, causing photoablation (cells hit by the ray are vaporized and expelled); this happens for the molecular dissociation at infinitesimal levels, with no damage for adjacent cells.PRK is especially effective for treating presbyopia: the doctor removes the corneal epithelium, that is the most peripheral portion of the cornea, obtaining a curvature. Is is also used as a treatment for correcting hypermetropia and myopia.
PRK is performed in the clinic: patient is admitted and discharged without being hospitalized. Laser surgery requires just a couple of minutes and patients are awake during the procedure. In some cases a sedative can be given orally. The majority of patients don’t experience any pain sensation during PRK: the eyes are under the effect of an anaesthetic eye drops. Patients lie down on the ophthalmologist chait and placed right under the laser instrument before starting the surgery.
For great results, it is fundamental to rely on a high quality excimer laser. Some lasers are best to cure myopia, some others are more appropriate for treating hypermetropia and astigmatism. Dr. Badalà uses last generation lasers to cure presbyopia, in Milan and Catania; these lasers feature eye motion control (eyetracker) and pupil recognition.
It is a laser surgery for myopia that tries to combine advantages of PRK with the ones of LASIK. The superficial corneal layer, called epithelium, is removed using a very superficial microkeratome; the excimer laser is applied and then the epithelium is put in its original place.
The ideal surgery for high degree astigmatism and myopia consists in implanting intraocular phakic lenses (IOL), also called intraocular contact lenses. This special lenses are made of silicone or collamer, and are pretty useful to correct vision flaws like myopia, hypermetropia and astigmatism, even if it’s high degree (10-25 diopters).
These lenses are similar to classic contact lenses, but are implanted between the cornea and the iris, or right beneath the iris.The innovation consists in achieving the same results of LASIK, without having the need to remove eye tissues. In some cases, the phakic lens implant and LASIK can be combined to achieve the optimal correction.
Implantable lenses are suitable for patients that cannot achieve the same results with other procedures; this may happen for example in case of very thin cornea or high degree astigmatism (keratoconus). The kind of lens is fundamental to determine the final vision quality.
There are several kinds, but the most common for treating astigmatism are:

With a photosensitive lense, vision is measured two weeks after surgery, spotting possible flaws like myopia, astigmatism and hypermetropia; the lens is customised to the patient’s needs. Refractive Phacoemulsification surgery can be used to correct serious defects of myopia (more than 10-20 diopters) and hypermetropia (more than 6-7 diopters) for patients that are more than 45 years old. In this case, the natural lens, even if transparent, is substituted with an intraocular lens (artificial lens).
If last generation lenses are used, it can also be possible to correct presbyopia together with astigmatism. The cost of this surgery may vary depending on the lens implanted.
Astigmatism laser surgery is completely painless because anesthetic eye drops are very effective.
Surgery lasts for about 5 minutes and follows a short preparatory phase.After surgery, when the anesthetic effect ends,there could be burning sensations, light sensitivity and a light headache.
